Ankle and Foot Conditions #2

A. Douglas Spitalny, DPM

Rheumatoid Arthritis

Rheumatoid arthritis can be a severe debilitating disease crippling feet.   Women seem to be more affected then men.

RA can affect the foot in similar fashion as the hand, ulnar deviation --> fibular deviation.  RA classically will have joint destructive changes and eventual joint ankylosis.  Medial joint pannus with cystic erosions are classically seen at the MPJ's.

RA patients are easy to spot, severe bunion and hammertoe deformities.  Often toes are dislocated.  Patients often feel and look like they are walking on glass.

RA can often affect the rearfoot and ankle.  Oddly, the talonavicular joint is often involved more frequently.

These feet are at least salvageable

With this type of foot, metatarsal head resections are in order

The deformities with this foot are so severe that attempting to restore it is almost impossible, in this case I performed a transmetatarsal amputation
